You can use this form for two reasons:

1. To request the 'Right to be Forgotten'. If you ask for this then we will erase all personal data concerning you from our systems without undue delay.

2. Submit a 'Subject Access Request'.  If you ask for this then we will provide you with copies of all information that we hold on you.  We are legally required to do this within one month.
